New Customer - Existing Home Application

Below you will find the application to start utiltiy service for customers who have purchased an already existing home. In addition to the completed application, a copy of the settlement statement is also required. This copy can be attached to the completed application below (preferred), emailed to city offices at utilities@rivertonutah.gov or faxed to 801-208-3197. If the settlement statement is to be faxed or emailed and the application is submitted online, please enter the customer name and service address in the subject line of the email or at the top of the fax.
